Death Certificate Amendment

Purpose:

Case specific form used to enter death certificate amendments

Navigation Tips: By clicking on the Create Request button or the Add Response button, the user will be taken to the popup to input Requested information and/ or Response information. The highlighted yellow fields are required fields.

• Press button to create new amendment. This will bring up popup that allows user to enter amendment data, date is required and auto filled.
• Sub manner of death list is dependent on manner of death field selection
